In this case, the unusual thing may be that I experienced and witnessed the process of interprofessional collaboration between community nursing and other professionals I had never known about before. This event made me realize that there are many aspects of community nursing that I knew before and in this situation it is the importance and responsibility of interprofessional collaboration. From my nursing theory course I learned that interprofessional collaboration occurs when the nurse establishes relationships with other professionals that allow them to achieve a common goal to provide care and strengthen the healthcare system and the clients involved in it. (Betker & Bewich, 2012, p.30) In this case, our common goal is to provide the appropriate care to patients/residents so that they can restore their health after hip or knee surgery. The textbook on nursing leadership and management states that “interprofessional practice removes the gatekeeper and allows the client access to all healthcare professionals based on the skills needed.” (Kelly & Crawford, 2013, p.35) At this event, my preceptor and I gained knowledge about Revera and will pass this information along to patients who are interested in staying in a nursing home after discharge from the hospital. One literature talked about how, according to the Institute of Medicine, it is critical to have the ability to work together as part of the interdisciplinary team to assist in providing high-quality patient-centered care. Furthermore, effective collaboration between healthcare professionals results in improved patient care and outcomes.
